The website includes an optional patient feedback survey. By submitting a response, you grant GI Ready, LLC a non-exclusive, royalty-free, perpetual, worldwide license to use, reproduce, modify, and analyze your response for the purpose of improving the educational content and the website. You warrant that your submission does not contain patient names, dates of birth, medical record numbers, or other Protected Health Information, and that your submission does not infringe the rights of any third party. GI Ready, LLC reserves the right to redact, hide, or delete any submission at any time, in its sole discretion, including for the protection of patient privacy or for any other reason.
7. Authorized Surfaces
The website includes both patient-facing educational surfaces and operational surfaces intended for use by authorized clinical staff. The operational surfaces are intended for use only by authorized clinical users at participating practices. Unauthorized access, automated access, or attempted enumeration of the operational surfaces is prohibited.
8. Intellectual Property
All content on the Site — including text, images, layouts, HTML, CSS, and downloadable PDFs — is owned by GI Ready, LLC or its licensors and is protected by United States and international copyright law. You may view and print content for personal, non-commercial educational use. Any reproduction, modification, redistribution, public display, or commercial use of the content without prior written permission of GI Ready, LLC is prohibited.

These Terms are governed by the laws of the State of Indiana, without regard to its conflict-of-law principles. Any dispute arising out of or related to these Terms or your use of the Site shall be brought exclusively in the state or federal courts located in Allen County, Indiana, and you consent to personal jurisdiction in those courts.
